i noticed that happen when you get KOed and respawn in your very near HS.
if the HS is in your loaded land your character will not update to take out stars from head. you need to port to boston or log out and rejoin to make that disappear
idk if is a bug, i noticed it works in that way for me ^^

A zombie infestation seems to have struck Roanoke.

I've seen this too.
I suspect this is the same bug that causes the skin texture not to disappear from corpses after skinning them. Moving in and out of a building will fix it.
