Offline insanity

Is there any way to receive insanity while character offline except using cursed teddy bear? I have an alt, this morning I log in him and got 6 levels of madness. I smoked hookah, lowered it to 5 and logout. Now I log in him again and got 7 levels of madness. It seems like tomorrow he will wake up already dead.

Do you stay on long? Maybe rack up some insanity and not notice?

Perhaps some crabs on a stick nearby?

Maybe cursed? I would go to the priest and cleans yourself. Possibly get a new teddybear too.

I have one crab for testing. Is it affects on offline characters?
