i guess if someonekept mining the black walls it would happen. Normally people just go down to the next level.

This is an intended feature of the game, I myself have a maze for anyone that gets through my defenses.

With over 45,000 different combinations of finding the correct path of mineshafts that leads to a treasure room. I'm happy to say its very effective

Yeah, cavedowns are very common in the shallower levels, that is not unusual if you keep mining out black walls.

btw does they all lead to one level? or every ladder have their own level?

Each ladder goes to its own level. As stated above this was intended so players could make quite complicated 3D maze structures in their mines.
