and a another pot willowtonik without cure, i hate this feature :/ .... murder and permadeath ok, but unknown diseases based on bad sounds and animations?

Try leeches then, maybe you are mistaking headaches for coughing

took me 12 sips of tonic to clear my first disease, and that was without any incorrect remedies applied. Sometimes the numbers just don't roll in your favour.

Big Pox, cured with Willowbark Tonic, main symptom is coughing, animated with sound.

Frontier Flux, cured with Columbine Paste, main symptom is vomitting and has a animation where your character flinches, faces the ground and vomits with sound.

Land Legs, cured with smelling salts. Main symptom is dizziness though the animation has no sound and is silent. The animation itself is the pilgrim looking dizzy/losing balance/wobbling on its feet. Watch your character closely and count up how many times each symptoms happen.

Maroon Fever, cured with leeches. Main symptom is headaches and the animation of such is the pilgrim putting a hand to its head and make a sound of exhaustion/pain, can't describe it well.

Based on your posts you either have the maroon or pox, but then you mentioned losing phlegm, so it's tricky.

Every time it drains a bit more blood it probably rolls its chance to cure you. Just keep it on till it's full and then swap if you think that's the right one. But I do suggest you just keep an even better eye on symptoms and see which it is, perhaps go to town and ask people for indication if what you thought was a cough/headache is actually that.
