Series did a amazing job until the last season and a few minor things in the previous ones to keep true to the books

It's all by design. Mr. Martin has said there's a few things he'd have done differently if I recall right, plus, some of the stuff in the books don't make for good TV. Hell, even Peter Jackson had to take a few creative licenses to make the Tolkien movies, and his mantra was "no changes." Sure, you get some nerd fights over why he did it that way, but he was trying to make 3 3 hour movies, not 6 4 hour movies.

And my prediction: Jaime Lannister will only die to his own hand (or suicide by death wish type thing). That kind of guilt just lives forever.
Unless the fallout from book 4 is what does him in. Been too long since I read it now and forget what he was doing about Cersei.
