After I've gotten a disease I'll actually let a rat bite on me for a minute or two to build the debuff up a third of the way or more to increase frequency of symptoms. Once they happen once a minute it becomes quite obvious which symptom you favor.

keep in mind that, even if you guess correctly which disease you have, it can still take multiple doses of the correct medicine to succesfully cure you.
Also keep in mind that using the wrong medicine will lower the chance that the correct medicine cures you - so take care!
