My favorite so far is the soot inspiration which is practically invisible in an inventory slot.
I don't mind being penalized for making a stupid decision (swimming withouth looking at Phlegm or attacking a snake without enough Yellow Bile for bullrush to make a one-hit kill); however, I get a bit upset when the complexity of the UI makes it very easy to make mistakes when one is tired or distracted (like the severe overloading of left and right click actions).
