LukeSkywalker, you are correct, there ARE some minor inconsistencies with the madness, whether or not by design.
During the last blood moon, I had been on for several hours when it hit. No change in madness levels. I immediately sharpened a dozen sticks, and put my character to bed for 8+ hours, then later used him for several hours no ill effect, no madness levels.
After putting the first character to bed, I woke a second alt, and used him during bloodmoon for several hours, no ill effect, no madness levels.
When putting a character to sleep in a BED, you sometimes wake up with an additional madness level, for no apparent reason, if the character was in bed a short period of time (less than 8 hours). A simple LOG OFF seems to not have this (accidental?) penalty. When exchanging items between characters, I have learned not to put them to bed, but log them off where they stand.
Same is true of TP'ing to town. You can pull a fresh character out of a bed, after 8+ hours sleep, theoretically at zero insanity because of the bed-rest, and you'll (rarely) gain a madness level (I would guess at about a frequency of every 20th trip to town).
Another thing that causes a madness level, on occasion, is using your torch to light a fire in a mine. I have learned to light fires in a mine with tinder-drills. Or else, sharpen a boatload of stakes after lighting the fire.
BTW, I am nearly 70 years old, and I am extremely unconcerned with anyone's opinion of my veracity.
Also, just had my first permadeath. Was carrying a bolder past a coal clamp, and evidently got burned. Took him a couple days to die. He was at madness level 2 when I put him to sleep. A day and a half later, after the bloodmoon had started, I woke him to check him, and there was a scream, and an announcement that he was dead. After a few seconds, the screen cleared, and he was gone. (Update: The alt died because I looked at him during the bloodmoon. ANY KO outside town will result in death, during bloodmoon, and my char was in a permanent KO cycle.)
In Retrospect: There is NO "burn injury". I am convinced the injury was due to an unknown (at the time) bat, acquired in a level 1 mine.
