Unsure of what happened

So i was messing around swimming inside of Boston and i got knocked out due to phlegm depletion and after a minute or so i just died? Last i checked only a player could do that and outside of Boston...then when i went on an alt acc back to my claim my body was still intact and not a skeleton and i could interact and l3wt it what exactly happened?
