Don't like the axle breaking and perma-death idea. How about we settle on broken leg? Or how about squashed toe causes you to occasionally limp stopping all movement for 1.5 seconds. Only fix is to equip a crutch made from leather, cotton stuffings, and two oiled boards. Also if the toe isn't fixed with crutch in two hours it becomes infected and requires an ointment from gelatinous lard and corn oil. If the toe isn't fixed in a day then ganggreen sets in and you have to amputate the leg with a metal saw and use iron tongs to cautorize the wound. Then a peg leg has to be crafted with two oiled boards and a blistersteel bar to walk without a crutch.
Edit: Peg leg can be stolen.
