I play quite a lot but you are right in assuming this would likely not impact me too much as I don't keep piles of plague rats with my food and the people I trade with are smart enough not to keep plague rats with the items they sell. Also it would not hurt trade with trusted merchants as selling infected goods would hurt their reputation so they would take care not to, it would be yet another service they offer, it might hurt trade from anonymous stalls but even selling infected goods though a stall would be bad for the owner as people would eventually figure out which stalls sell clean items and which don't. As this would only effect people that did foolish or malevolent things with plague rats the main thing that this would likely do is allow people to bribe shadier merchants into slipping some infected food into a shipment for another town. It would be interesting if a clever attacker could force the defenders to deal with an outbreak or track down and eliminate the disease while trying to prepare/deal with a siege.
While this could create a large scale and hard to eliminate outbreak I doubt it would happen as it would require massive incompetence or maliciousness by large trusted merchants which would destroy their reputation. Accidental outbreaks would likely be limited to a single group and possibly its trading allies and could, assuming no one is being intentionally deceptive, be quickly tracked down and eliminated.
Also it would be interesting if witches could create special diseases to spread like this. The ability for witches to spread special more painful and harder to cure disease would create even more distrust between witches and the good Christians on Salem.
