There's a few ways, but the most direct I can think of is right click on the desktop, open Properties, and go to the Settings tab. A new window will open and there should be an "Adapter" tab. It will list the details of your video card. On the Adapter tab, if you click the Properties button, it will open up information on the video device. You can select the Driver tab and it will show the version of driver you're using.
So, to sum up, provide us with
Device name (can be found both on the adapter tab and in the device properties window)
Driver version (can be found after opening the device properties and selecting the driver tab).
