Indeed it does. You're technically still on the water tile when you're climbing the cliff, so you just keep losing phlegm. Considering the time it takes to scale some cliffs, it's quite possible to drown before you reach the top.

You should perceive it as if that were a waterfall. That would explain such phlegm drain and why it takes so long to get to the top. Try climbing up a waterfall in RL, and you'll see how forgiving in game it actually is.

I'm not rationalizing the bug (it's happened to me; I almost drowned when I realized what was happening, and there's no way to interrupt climbing afaik). I'm just stating what I believe causes the bug to occur.
