I think some of that has to do with the games publisher/management. Some companies don't put up with it and will quickly permaban accounts that get complains of any severity level. Three Rings (Puzzle Pirates, Spiral Knights) won't put up with any and will issue a 30 day ban on a first offense and a permaban for the account on the second.

Look guys, the women on those sites are the most vocal, not necessarily representative of the whole gender, and probably had some experience that they have decided to take out on all of you, so please dont tar us all with the same brush.
I know I am one of the less common female gamers, but I understand that this is a male dominated world, and I dont want you to change how you are just cos I am here.
Just accept that we are all basically the same, with some evolutionary biases, and that at the bottom line we are all human, we all have feelings, and we can all be hurt, sometimes by the stupidest of things.
I think the biggest problem is that on line the person on the other end... doesnt really exist? Its just words on a screen, and so we can discount their feelings and do what we want.
Fact.. its not a nice world out there, and at some point we have to man/woman up to it and filter out the real **** from the bullcrap that doesnt matter.
Dont sweat the small stuff.
(and yes i agree that there is a movement towards it being acceptable to laugh at mens pain, and its wrong to laugh at ANYONES pain)

I'm not close minded, I believe the girl gamers should be giving back as hard as they get - Misandry (hatred of men) jokes - can't think of any (I'm hopeless).

What a load of rubbish. From my experience, girls in mmo's often get treated kindly with extra special treatment. They love it when guys give them attention, that's why they play those games. And every guy is usually a suck up when they find out 'oh so there's a girl in the guild?!'.

Couldnt expect anything mature to come from console gamers though because typically they're young schoolkids and well if you throw girls into the mix of course there is going to be insults. Its a playground without authority.

I think it's mostly just plain old trash talk being taken the wrong way. "I'll ***** you up!" is pretty tame when said to a guy, but I can see how a girl might misinterpret it.

I think it all boils down to your first point: the men that use such abusive languages on girls/women abuse other guys to the same extent (except for other choice of words), while those that don't really **** on other guys won't do so on girls either (matter of speaking). Personally, I might be biased since I make a point of never to talk to someone in a way I wouldn't want to be spoken to.

A contributing factor might also be the immature teenagers, because -let's face it-
