Yea there are mechanics with smelting i never went into extreme testing with, but im sure to make bars with a 1:1 ratio will require you to burn either full furnaces of ore or lime first, then on the next cycle, you will run high quality ore / silver ore with a 5 ore : 20 lime ratio. (Untested by myself and somewhat a theroycraft based on old results when i tinkered with smelting 6 months ago)

When i got smelting to a ratio of 2:1 for my silver / high Q metal (ore:bar) i stopped further testing as i was happy with these rates. Im sure if i went further i could have found the 1:1. I got my 2:1 ratio without needing to burn full runs of just ore/lime only.

It i am correct in my theroy, i doubt it is a bug, due to you needing to burn a cycle of ore/lime first, then yeild the amounts on the next cycle from the excess pool, only way to really fix / stop this would be to totally change smelting so that nothing is left over at the end of a cycle to give a boost on the next cycle.

Hope i do not upset people with my theroy crafting here.
