So far I have not seen any indicator of madness. I've murdered dozens of turkeys, rabbits and afked for a long time, but still nothing. Maybe I'm just bringing down my madness by doing stuff? If so IDK why people are complaining, you literally don't even need to try. Unless its much more difficult in the darkness.

A few suggestions:

How about seeing animals fight eachother increases insanity? I would say little to medium amount, since snakes vs rabbit is a common encounter.
Wondering and exploring reduces insanity?
Getting a Cross Shaped Splinter when cutting a tree should modify insanity? your choice, up or down
Finding a nest should increase insanity.
Make it possible to sweeten tea (milk in future) with honey and when consumed reduces insanity?
Hammocks to reduce insanity, as beds increase inspiration?

Idea: Spending too long sitting down could lead to a PE where your foot falls asleep and you get pins and needles :lol:

if you crash or lose connection carrying something you spend the whole 40s losing yellow bile... I think I lost about 100 when my Internet came back I was still carrying a boulder and yellow bile almost gone...

My only complaint about this patch is that stimulants and poisons (ie tobacco), in general, do not decrease "sanity" behaviors, and instead usually agitate them. Burning various forms of incense, however, would be ideal (such as peyote or hemp buds, sandalwood, rose hips, etc, etc).

I saw that comment earlier about madness from gluttony. Even though overeating like this is itself a bit of a mental disorder, when was the last time you heard of someone going mad when they were so stuffed they'd explode if they fell out of the chair they were sitting in? The opposite is usually true--they're so sated (and bloated) they can't do much anything. If anything, eating excessive amounts should give a performance penalty, so you have to decide whether to pig out or do some work for the next couple hours.

Frequent use of tabaco should lead to to addiction, where an addicted character would gain insanity if it doesnt smoke for some time.
So if you keep gaining madness levels fast and then counter them with smokes you will end up in situation where you have to keep smoking more just to maintain your sanity, or perform more insanity-lowering actions until you get out of the addiction

I was looking more at contemporary cigarettes, dunno how it works with tobacco. People who smoke a lot get very nervous if they can't smoke for some time.
