Does it depend on biles?
How much actual insanity before you permadie?
According to the alt testing by rubberduck. It didnt seem like he got past madness level 1?

wow... this is the first time i seriously considered quitting Salem... this patch was a very unpleasant surprise

Permadie at Madness Level 10
Does not depend on Biles
Insanity itself is not necessarily what kills you. As long as you keep your insanity in check you will stop gaining madness levels which will then stop you from dying.
I do believe the issue with Rubber Duck is he was using a custom client that had not been updated yet therefore was using static images for the masks.

Now we are forced to wait the 24h out with ACTUAL playtime instead of afking for the duration of the debuff.

Indeed. And countless other benefits of holding players accountable for actually playing Salem when they are playing Salem
