Is it possible that diseases evolve from one form to another by itself? I found very hard to recognise illness at start, until bar get at leats half symptoms are very random for me.

I have never observed a case where the disease changed in a patient and its most likely not possible. Early on its easy to miss a symptom due to how short they last, which adds to the complication of correctly diagnosing an illness. When infected, if you want to quickly diagnose the illness, its best to play zoomed in with your character facing the camera so you don't miss any symptoms. Also if you have trouble keeping track of all the symptoms that have occurred record them on paper or a text document, the primary symptom can usually be identified with in 10-20 observed symptoms but due to RNG the most of the symptoms my not be the primary one.
My general rule for diagnosis is that if after observing 10+ symptoms if the most common symptom occurs 40% or more of the times and the second most common symptom occurs less that 30% of the time you have found your primary symptom. If these thresholds are not met then continue observing until they are. Also it rarely takes more than 5 doses to cure a disease and only once has it taken me more than 10, so if you take more than 5 doses(10 if the cure is easy for you to make in large amounts) without curing a disease throw away the data and start the diagnosis over.

I have a picture of a rat, and it says diseased on me, i dont know how i got this...
i am brand new to this game.
every once and a while my charachter moans..help!

The cure you require depends on which of the diseases you have. There is no real easy way to diagnose this though, you have to check for symptoms - each disease has a symptom corresponding to it, and this sympthom will occur more than the others.
I have neither the crayons nor the time to explain it to you.

The act of they symptom happens so quickly , it appears to be moaning, and now i just got what appears to be a vomit.

yes- symptoms continue for longer as your disease grows worse over time.

The symptoms are: vomiting, dizziness, coughing, headache as seen in this post

well i caught a small graphic past the picture of the rat, and when my guy groans it says "stunned"
