You may not be notified immediately if you gain a lvl, only if you do something that effects madness,
like, doing something that actively adds or removes insanity will force an update of your madness lvl.
But you won't be at lvl 2 and then 8 without getting an update in between as that takes a very very long time of passive gain.
Some explanations:
1.) You did not actually log out due to whatever reason and stayed online for a long time, the gain would be quite typical for that time frame.
2.) Some playful dev teased you, did you argue with some of them lately? ^^
3.) Someone else logged onto your char.
