Two Questions

#1 What's the deal with the Hookah in the crypt? I thought you could only access the hookah by going into the den, why has one been placed in the crypt? Also is it safe to use, for instance is PVP or something like that enabled down there or is it all kosher?

#2 How do you use the tiny pocket watch to move faster? Do you simple equip it in your hand and how long does it make you move faster for? Also does having run enabled make you move even quicker?

#1 It was placed there some time ago. It's safe other then the risk of disease from freshly dead corpses if you don't have a plague mask

#2 Right click, and click "I'm late" it will consume the watch, and you will move faster. If you want more specific timing and interactions I'll encourage you to test that on your own.
