How often can I smoke the hookah, 24H ?

yes each 24h one smoke down 1 madness

But make sure you do wait at least 24 hours. If you go too soon the timer will reset AND you won't get the benefit.

I may add, the timer is exactly 24h iirc, and if you use any hookah again in those 24h, the timer will refresh without you gaining (or losing, madness that is) anything.

Also, the longer you use it, the higher the inspiration you gain, and the more risky of getting stunned and getting nothing.

I recommend using the timers in latikais client to make a 24h timer (that sticks to ingame time, important in some cases).
