You take damage when you find entrance to the next level. Damage depends on which level it is and increases with depth.

The bats didn't jump on me since I was already KO'd when I spawned in there due to taking too much damage because of the cave down, lucky me

When you mine the black tiles, after softening, there is a chance you find the entrance to the next level. If that happens you are immediatelly dropped into lover level, take blood damage, and bats spawn around you. The tile you mined on previous level becomes entrance to the lower level, that you were dropped to.
