there will be 3 coffins.
in each coffin is a chance to get your soul back.
if you chose the wrong coffin you die

2/3 coffins are the death scythe
1/3 either your soul or treasure + free round

I'll piggyback and suggest: what if when someone died and their scalp score and time played were high enough, they would have a corresponding number of gold fillings that you could pry out with iron/hellfire tongs?

Apparently those were being used in the late 1400's for wealthier people.

Even better, have large gluttony sessions have a chance to give charters cavities, that they will need to fill or take gluttony penalties and suffer from increasing pain. And let them be pulled out of any dead or KOed character.

Just have the gold fillings be in there somehow. Like how needles always seem to get into haystacks.
