I, as a trader, had many newbie customers, and the vast majority of them had at least 1 or 2 madness levels, and problems to get back to zero, also, a steep learning curve is not a bad thing, but some, both newbies and returning players, were caught off guard by it. One did not even know about madness until I told him and he had lvl 6, ok, that might be a little extreme and rare, just saying this happens, too.
Also, I had one customer buying a pickaxe, just to sell him another one the next day because he lost the first one to madness, which quite a harsh setback for a new player, and that can even happen at madness one.
I for myself never gained a lvl of madness myself, but I take care to make a studying-break about once an hour, and usually do only sane things in the meantime anyways.
Also, when JC gifted me some firsthand experience with madness, I found it quite hard to get back to zero madness, I took 3 of the best tobacco and the hookas while still staying at madness 1, only the teddy did the trick here.
I don't know what to really think of the madness mechanic, for older players who know about it, it is probably not an issue, for the newer players though, it might be a questionable experience to "wake up" (meaning, realizing there is madness) and find out they have a serious problem.
Those who manage to deal with it though, are probably worthy of continuing to play this game!
