I went to sleep in a bed with a teddy bear in hand with madness level 1 at 11 pm. Did not log in until 6 pm tonite after work. Madness level is still at 1. ?

Where do you get the number "1" from? The humour interface on a modded client?

two notes:
- the client typically takes a while to report your actual madness level. if you do not have the lowest madness level, it'll show up as a system message relatively soon after logging in
- If you are using my client, the humour bar will show madness levels from 1 to 10 (well, 1-9 and then X) - 1 is the lowest you can go. IIRC, it'll actually show madness: 0 when hovering over it, if that settles your mind.

On topic, I feel the 1-X is a bit thematic. I also kind of like the X when you're about to die, and I feel zero-indexing is not a good idea for most people (although maybe for this, it might be).
I haven't had much comments pro or con, so I went with it. Originally I didn't even think about it when doing the images (they're probably actually called madness0.png through madness9.png though, come to think of it).

I don't know. Zero level of madness is much more clear condition, then 1rst level of madness. At least for me and for players I have discussed this with.

Yes in enders client was madness level one. It shows when you hover over the top humour bar. The rest of the stuff being said I dont understand. I thought the teddy would lower madness by 1 after 17 hours.

If you are using a custom client and it says you have level 1, that means you have no madness. There is nothing to remove.

When I do not have madness it shows as level 0. With level 1 there are psychotic episodes.

Try it one more time. Most likely you are doing something wrong.
